 UV Tanning bed client waiver and consent form

Acknowedgment & Consent

I understand that exposure to ultraviolet (UV) radiation from tanning beds may cause skin damage, premature aging, eye injury, allergic reactions, burns, and may increase the risk of skin cancer. I voluntarily choose to use tanning equipment and services provided by TYC Glow & Wellness.

I certify:

• I am at least 18 years of age or have a legal right to consent to tanning services

• I understand that overexposure to UV radiation can cause serious injury

• I understand that certain medications, cosmetics, or health conditions may increase sensitivity to UV exposure

• I agree to wear protective eyewear during every tanning sessions

• I understand that results vary and no guarentees have been made regarding tanning outcomes

• I agree to follow all posted rules and staff instructions regarding tanning times and equipment usage

• I acknowledge that it is my responsibility to inform staff of any concerns prior to my session

Liability Release

I voluntarily assume all risks associated with UV tanning and release, discharge, and hold harmless TYC Glow & Wellness, It's owners, employees, contractors, and affiliates from any and all liability, claims, damages, or causes of action that may arise from injuries, damages, allergic reactions, burns, or other complications related to the use of tanning equipment or services, except where prohibited by law.

I understand that tanning services are cosmetic in nature and are not intended to diagnose, treat, cure, or prevent any disease.

Salon Policies

• Equipment is sanitized by staff between each use

• I must notify staff immediately if equipment appears damaged or unclean

• Failure to follow salon rules may result in refusal of service

• Packages, memberships, and sessions may be non-refundable unless stated otherwise

• USING OILS, OUTDOOR LOTION, OR LOTIONS UNAUTHORISED BY A CERTIFIED DISTRIBUTOR ARE PROHIBITED.

  • If found using prohibited lotions may result in Barr from our salon and may result in fees for damaged equipment

• I understand that by not notifying staff prior to my session of any equipment damage may result in me being held responsible for equipment repair costs

Virginia UV Tanning Disclosure

Virginia law requires customers to be informed that exposure to ultraviolet rdiation may increase the risk of skin cancer and eye injury. Protective eyewear must be worn during entire tanning sessions.

I have read and fully understand this waiver and consent form. I voluntarily agree to all terms stated above.

I certify that I am the legal parent/guardian of the minor named above and consent to their use of tanning services.
